Patricia, Thank you, great points. Let’s expand them a bit more. This week we are to examine the pathological osteoporosis processes. The disease is increasingly being recognized that multiple pathogenetic mechanisms interact in the development of the osteoporotic state. Bone is continually remodeled throughout our lives in response to microtrauma. Bone remodeling occurs at discrete sites within the skeleton and proceeds in an orderly fashion, and bone resorption is always followed by bone formation, a phenomenon referred to as coupling. Because bone development starts at the time of our birth how would you treat your pediatric patients? How would you prevent the pathological process you described?

I just want to make a point about the importance of having an in-depth understanding of the pathological processes we are trying to apply in this course. If you can drill down to the cellular level instead of a broad stroke of the process, your diagnosis and treatment plan will be more accurate. There is a certain combination of clinical signs and symptoms that present depending on the cellular and chemical reactions occurring in the disease process.  One step of the inflammation process is the vascular permeability that occurs with the initial insult. This results in the swelling. In the abdominal system this could present with a tender rotund abdomen or diarrhea because of the body’s attempt to rid itself of the excess fluid. How would you treat this?  I am not so concerned in this course with the treatment, but your ability to drill down and connect the clinical symptoms with the pathological process. To expand your thoughts a bit more. One of the objectives this week was to examine the two main types of gall bladder diseases, cholelithiasis, and cholecystitis.  Cholelithiasis, as you know, are otherwise known as gallstones are hard, pebble-like formations that develop in the gallbladder.  Cholecystitis can be chronic and acute. It is defined as an inflammation of the gallbladder.  Other gallbladder diseases to consider are polyps, which are a growth of tissue in the gallbladder and do not forget cancer tumors.  Abnormal growths on the gallbladder and bile ducts are common cancers and must be included in your differential diagnosis.  The same symptoms occurring with the above can also be found in biliary dyskinesia. This is a condition where bile does not flow from the gallbladder or common bile duct and backs up in the gallbladder. Patricia, Great points as you also noted there are various methods of classifying anemia. They are usually classified by their causes or by the changes that affect the size, shape, or substance of the erythrocyte. As the group has noted symptoms of decreased oxygen supply are abdominal pain, nausea, vomiting, and anorexia along with a low- grade fever (<101F). The results from a release of leukocyte pyrogens from ischemic tissues. Would you see these clinical symptoms in an acute onset or chronic situation?

Anemia is a disease commonly seen in everyday medical practice, its occurrence accounts for around 2 to 4 billion cases globally due to vitamin deficiencies, chronic diseases, infections, and hemoglobin disorders among others. It reduces the number of red blood cells and hemoglobin jeopardizing the body’s oxygenation compromising several organs ( Jansen, 2019).

As per the present case scenario about a 50 years old patient presenting with fatigue, pallor, dyspnea on exertion, and palpitations, and a laboratory report of a low hematocrit 32, hemoglobin 9.7, and reticulocyte count low at 47% MCV of 78 and MCHC of 32.1. The individual s/s is due to a decrease in circulating oxygen that affects the heart, arteries, capillaries, and ventricles because the drop in RBC’s generates as well a decrease in blood volume that is compensated by the movement of interstitial fluid into the intravascular space expanding plasma. Thus, the heart cardiac output is affected to respond to the increased in metabolic needs which can lead to heart failure. In the pulmonary system, this increased demand generates an increase in the rate and depth of respiration where more oxygen is released generating shortness of breath, fatigue, dizziness (McCance & Huether,2018). 

Proper diagnostic testing results vital; the patient’s hemoglobin (hgb) is low, as well as the mean corpuscular volume (MCV), and MCHC. Hence to further determine the etiology of this problem additional biomarkers will be checked: A complete CBC including, hemoglobin and hematocrit to determine the number or circulating blood cells. Iron in conjunction with transferrin, soluble transferrin receptors ( to differentiate the type of anemia), transferrin saturation, Mentzer index ( to differentiate between iron deficiency and beta-thalassemia), ferritin, vitamin B12, to assess for iron deficiency; reticulocytes ( chronic anemia), hepcidin-25, bioactive ( hereditary anemia, iron deficiency by the liver), hemoglobin electrophoresis ( suspicion of microcytic anemia) and mean corpuscular hemoglobin (MCH) to determine hemoglobin mass per RBC (Jansen, 2019).

Many of these remodeling effects seem to be mediated by transforming growth factor. This results in a reduction of perfusion to the skeletal muscle causes atrophy of the muscle fibers.   • Let’s expand your thoughts a bit more…Most cancerous breast tumors are fixed, hard, and solitary. These are also typically non-tender. The sooner cancerous tumors are detected and treated, the better the patient outcome.  So the guidance notes that self-exams do not improve detection, neither does increasing the frequency of mammograms. So what have you found in your readings that would be the best advice for providers?

Patricia,Thanks for t he information you provided n your post. Let’s expand on your thoughts a bit more. In an autoimmune response, the healthy cells and tissues are destroyed, because the body’s ability to recognize itself is not working or malfunctioning. This results in the patient’s major systems malfunctioning and not able to function.  The pathology involves T cells and autoantibodies that create the autoimmune diseases. An example is Type I diabetes. What happens in this situation?
